I hereby release Healthy Epic Living and any of its volunteers, and/or affiliated organizations from all actions, claims, or demands that I, my assignees, heirs, distributes, guardians, and legal representatives nor have, or may hereafter have, for injury, death or damage resulting from my participation in the above described activities.

I AM AWARE THAT THE ABOVE DESCRIBED ACTIVITIES ARE HAZARDOUS ACTIVITIES, THAT I AM VOLUNTARILY PARTICIPATING IN THESE ACTIVITES WITH KNOWLEDGE OF THE DANGER INVOLVED, AND HERBY AGREE TO ACCEPT ANY AND ALL RISKS OF INJURY OR DEATH.
